Employee Coaching
Employee coaching is a step used prior to taking any disciplinary action. The Employee Coaching Form allows supervisors to provide constructive feedback feedback on behavior or job performance that is not meeting expectations. Performance Improvement Plan
The employee Performance Improvement Plan (PIP) is used either when employee coaching does not produce the intended results or when behavior or job performance is serious enough to require formal discipline. The form consists of a Level I Warning, a Level II Warning and a Level II Term Warning. Escalation beyond a Level I Warning requires consultation with the Human Resources Manager. Request for Termination
The Request for Termination Form is used when Performance Improvement Plan (PIP) warnings do not result in satisfactory behavior or job performance improvements. Termination recommendations cannot be submitted without first utilizing the PIP Form. All termination recommendations require consultation with the Human Resources Manager and Chief Human Resources Officer to confirm the level of action is warranted. Staff Additions and Alterations
FS100 - Request for Position Addition/Modification
The form must be completed to request a new position or to modify an existing position. Once completed, the form will automatically be routed for approvals by the Superintendent, Finance and Human Resources. Once the approvals have been received, you will receive an email notifying you of approval. Personnel Action Form (PAF)
The Personnel Action Form (PAF) must be completed to process New Hires, Transfers/Reassignments, Title Changes, Pay Changes, Budget Code Changes and Leave Status Changes. After the online form is completed it will automatically be routed for approvals by the appropriate administrators. Once the approvals have been obtained, you will receive an email notifying you of approval. Additional Pay Transmittal Form
The Additional Pay Transmittal Form allows supervisors to request additional pay for employees performing extra duty work outside of their regular job requirements. Notice of Employee Resignation
Resignations should be submitted in writing a minimum of two weeks prior to the date of resignation. After you receive a notice of resignation, please forward this information to Human Resources via email at HR@texanscan.org. A Separation Form is available on the Separation Notice page.
Employees can expect an exit interview questionnaire sent to their email by their last day.
